Lectures A. Immunohematology and HLA 1. Concept and definition of transfusion service, history of blood transfusion, basic immunohem concepts - antigens and antibodies in general). BG and BG anti, complement, agglutination, hemolysis, precipitation. 2. Group systems of ery and their anti-AB0H system, history of discovery, significance, heredity, occurrence. Subgroups and variants of the AB0H system, teaching of group properties. The Lewis group system and its relationship to teaching. 3. Rh group system. Other ery group systems (antigens, antibodies). Importance of blood group characteristics - distribution, importance for transfusion, immunogenetics and for hemolytic anemias (including AIHA). 4. Hemolytic disease of newborns (causes, clinical manifestations, treatment, prevention). Antigens of platelets, leukocytes, practical significance. 5. HLA system (genes, antigens, antibodies, laboratory diagnostics). 6. HLA system - practical significance in hemotherapy, in pregnancy, in relation to diseases, for transplants (organ transplants, bone marrow and stem cell transplants, donor registries, immunohematological assurance of transplants). B. Transfusion Service 7. Organization of the transfusion service, legislation, good production practice in the transfusion service facility. Information systems in transfusion service facilities. Blood donation - registers of blood donors, assessment of blood donor eligibility, mandatory examinations of blood donors, collection and complications during collection. 8. Blood stabilizers, preservation solutions, blood bags, production of transfusion preparations, including special procedures (deleukotization, washing), release of transfusion preparations and standard of labeling of transfusion preparations. 9. Special sampling - separator techniques, autotransfusion (types of autologous sampling, special work procedures for work with the donor-patient and his blood). Therapeutic apheresis, stem cell transplantation, umbilical cord blood. 10. Transfusion products (their specifications, conditions of storage, delivery and transport) and blood derivatives. Post-collection exam of donors, release of transfusion products, control in transfusion service facilities (quality control, bacteriological control, internal quality controls). Prevention of transmission of infectious diseases by transfusion preparations. 11. Clinical transfusion practice - purposeful hemotherapy - indication of blood transfusion. Responsibilities and work content in blood stores, transfusion commission, contact between the blood store and departments, pre-transfusion exam, uni donor, uni recipient. 12. Administration of blood transfusion (preparatory work in the clinical department, administration technique). Post-transfusion reactions and complications, hemovigilance. Massive transfusion, transfusion during mass disasters, crisis transfusion centers. A Lab exercise 1. Familiarization with the workplace of the Transfusion Department. Principles of work in the immunohem lab and work with biological material, OSH training. Diagnostic sera and type blood cells. 2. Complete exam of BG in the AB0 system. Exam of subgroups in the AB0 system. Exam of Rh D antigens, incl Dw/in. 3. Determination of antigens of other erythrocyte group systems. 4. Exam of antiery antibodies - screening, identification, evaluation. 5. Direct antiglobulin test, specification of erythrocyte sensitization. 6. Investigation of antiery antibodies quantitative - titration. 7. Examination of the universal donor. 8. Compatibility Test. 9. Exam for hemolytic dis of newborns. Imm antibodies anti-A, anti-B. 10. Exsanguination - selection of suitable blood, examination, preparation of blood for exsanguination. Post-transfusion reaction investigation. 11. HLA - lab Dg of HLA antigens of class I and II. class lab Dg of antibodies, lymphocytotoxic tests. 12. Credit test.
